Home Visit & Assessment
Home visit by the Geriatric Care Physician or the Elder Care Coordinator is an important step to know the elder person better and to plan a holistic care plan for him or her. It also help to enhance the care by direct observation of his or her social environment and to know the persons caring for them at home.
Home environment plays a crucial role for the wellbeing of an aged person. Their living environment needs to be assessed for their hygiene, safety factors and prevent falls and accidents since it can create morbidity and disabilities for that person. Home safety needs to be planed accordingly for their better mental and physical health. The following points needs to be taken care of during Home Visit and planned accordingly after discussion with their treating Doctor, Kins, caregivers and or other people taking care of the older person -
